胎儿期主动脉和肺动脉的正常通道即动脉导管，出生后及时未闭锁。主要表现为主动脉血分流入压力低的肺动脉内，肺循环血量增加，心室负荷加重，导致左心右心肥大以致衰竭。当肺动脉压力等于或超过主动脉压力时，左向右分流消失，逆转为右向左分流，临床上出现发疳，导致Eienmenger综合征。
