I believe, that her photograph should be at the top of the back cover, with this message underneath it and then my name.  

 

Also, for the picture of myself at the end of the book, a photographer took it - and Stephen said it would be ok if we gave the photographer credit, by using his name in tiny letters either on the side of the photo, or underneath it - his name is Daniel Brennan.  He is nice enough to let me use it.
